How to Fill a Government Job Application Form Correctly

12 Oct 2026

Selection boards for popular government posts may receive tens of thousands of applications. The first round of screening is usually about the form itself: is it in the right format, complete, signed and supported by the correct documents? Many capable candidates are rejected at this stage without anyone ever reading their qualifications. Here is how to make sure your form passes.

Before you start

  • Get the official format from the notice or the institution's website. If you need to type it yourself, copy it line by line.
  • Use clean A4 paper, one side only, unless the notice says otherwise.
  • Decide whether to handwrite or type. Typed forms are easier to read; if you handwrite, use block capitals and a blue or black pen.
  • Keep your NIC, birth certificate and all exam certificates in front of you. Copy details from the documents, not from memory.

Personal details

Name with initials and name in full: write them exactly as on your NIC. If your certificates show a different spelling, mention it and be ready to explain at the interview – for example with an affidavit.

NIC number: double-check every digit. A wrong NIC number can make it impossible to match your application with your exam results.

Date of birth and age: calculate your age on the date given in the notice – years, months and days if the form asks for it. Mistakes here are very common.

Address: give a permanent address where post will reach you for the next year. Call letters for exams and interviews are often sent by post.

Phone and email: give numbers that will not change. Check your email spam folder during the recruitment period.

Educational qualifications

Most forms have tables for G.C.E. O/L and A/L results. Fill in the year, index number, subjects and grades for each attempt you are relying on. If the notice requires passes "in one sitting", make sure the table shows that sitting clearly. For degrees and diplomas, give the full name of the qualification, the institution, the class or grade and the effective date of the degree – not just the year you finished studying.

Professional qualifications and experience

List only qualifications relevant to the post. For experience, give the employer, designation, exact dates (day/month/year) and a short description of duties. Selection boards calculate experience precisely, so "2019 to 2022" is not enough. Attach service letters when the notice asks for them.

Language medium and exam centre

Many forms ask for the language medium for the written exam and a preferred exam centre. Choose the medium you are strongest in – you usually cannot change it later. Choose a centre you can reach early in the morning on exam day.

Declaration and signature

Read the declaration before you sign. You are confirming that the information is true and that false information can lead to disqualification or dismissal even after appointment. Sign in the same way you sign official documents, and add the date.

Attestation (if required)

Some forms include a section that must be certified by a head of an institution or another authorised officer, confirming your identity or that you are a public servant. Fill your part first, then take it to the certifying officer with your NIC. Do this early – officers may not be available near the closing date.

Payment of fees

If there is an application or examination fee, the notice will say how to pay it – often at a specific bank, post office or online. Attach the receipt in the place requested and write your name and NIC number on the back of it.

Ten common mistakes

  1. Using an old or unofficial format.
  2. Leaving cages blank instead of writing "Not applicable".
  3. Name spelled differently from the NIC.
  4. Age calculated on the wrong date.
  5. Results from different sittings combined when "one sitting" was required.
  6. Missing signature or date.
  7. Attaching originals instead of copies.
  8. Uncertified copies when certification was required.
  9. Wrong wording on the envelope.
  10. Posting after the closing date, or relying on ordinary post when registered post was required.

Keep a copy

Photocopy or photograph the completed application and all attachments before you send them. When you receive an exam admission card or interview letter weeks later, you will need to remember exactly what you declared. A copy also helps you apply quickly for similar posts in future.
